Fees & Policies

A credit card on file is required to book all services. Payments are due at the time of visit with debit or credit card (Visa, Master Card, American Express, or Discover) of your choice. 

A $50 fee applies to missed appointments. If you are unable to keep an appointment, please give 48 hours advance notice (Sundays and holidays excluded); otherwise, you will be charged for the time that was reserved for your appointment.  Example: If your appointment is scheduled for Monday at 11:20 am, you will need to cancel by Friday at 11:20 am in order to avoid being charged. Example 2: If your appointment is scheduled for Saturday 11:20 am, you will need to cancel by Thursday at 11:20 am. If cancelations are made with advanced notice, the $50 fee does not apply.

Insurance plans do not reimburse for missed appointment charges.  For your privacy, you must cancel your appointment by sending an email via the HIPAA protected patient portal, or by phone.

Medication refills are provided at the time of visits. In rare cases, if a patient runs out of medication, partial refills can be given to bridge the patient with enough medication until the patient’s next scheduled appointment. The fee for partial refills is $50.

Psychiatric Practitioner Cook sends electronic prescriptions to most pharmacies. As part of this service, Ms. Cook may have access to your medication history prescribed by other healthcare providers. Ms. Cook participates in patient drug monitoring programs (PDMPs). By virtue of being a patient of Ms. Cook, you agree to her having access to this service.
These classes of medications, which include Adderall, Ritalin, Xanax, and Klonopin for example, can be highly addictive. They can also be dangerous when combined with certain medications, illicit drugs, or alcohol. Thus, stimulants and benzodiazepines are prescribed conservatively and prescription monitoring sites are checked prior to refilling these medications. Lost or stolen prescription bottles of stimulants or benzodiazepines will not be replaced.
